Apply theoretical foundations and change principles while leading in the development and implementation of innovations that advance practice in a primary care setting

Outcome 8:

During my NURS 7430 EBP course, I completed an assignment discussing the Johns Hopkins Nursing Evidence-Based Practice (JHNEBP) Model and the model’s usefulness in utilizing nonpharmacological means of hypertension management. While developing and implementing my EBP project on hypertension management, I considered that theoretical foundations provide useful tools to improve healthcare quality. The model I chose to serve as a foundation for my EBP project was the JHNEBP Model. The model is an evidence-based approach composed of three interlinking components, including inquiry, practice, and learning. The model is beneficial because it ensures nurses are constantly questioning current practices and searching for relevant evidence to improve patient outcomes. Models incorporating clinical questions, evidence, and useful plan recommendations positively influence patients suffering from uncontrolled hypertension and many other chronic disease states. I utilized the steps of the JHNEBP Model when researching, designing, implementing, and evaluating my EBP project in the primary care setting. In NURS 7340 I completed two assignments where I researched several nursing and interdisciplinary theories/models and how they relate to hypertension management. These models can improve healthcare and enhance hypertension prevention and treatment strategies while searching for the best evidence techniques. The assignment taught me the importance of theoretical foundations and how developing and implementing innovations can advance practice in the primary care setting. Additionally, change principles are essential for NPs to address when providing education to patients. During a clinical encounter with a patient diagnosed with new-onset hypothyroidism, I was able to utilize my knowledge of change principles and theories to discuss necessary treatment and lifestyle changes. Working through the various stages of change I was able to create a patient-tailored treatment plan focusing on positive patient outcomes.
