During the PCNP program, health promotion and disease prevention is an essential NP role. Several clinical encounters during my clinical rotations were focused on providing comprehensive primary care, health promotion, and disease prevention across the lifespan. Attached below is my clinical tracking log portraying various patient populations seen while at clinical. Additionally, I have attached a pediatric, geriatric, and women's health SOAP note. During my adult and geriatric (NURS 755) clinical rotation, I was able to formulate and implement a health promotion project into clinical practice. After considering my prior knowledge and the high prevalence rates, I formulated a project to help patients with high blood pressure (hypertension) better understand their diagnosis. During project development, I created an informational flyer that was used to educate patients regarding hypertension disease process, risk factors, blood pressure numbers, recommended treatments, and the importance of diet, exercise, and weight control.
